Notes from the Chair

As usual, our Executive Committee meeting, held in Boston on October 26, 1996, was lively and informative. The content of our meeting was discussed subsequently with those present at the Business Meeting on Monday October 28, and I will summarize to update those of you unable to attend.

Our Section continues to grow, and we now have over 500 members. Our budget remains healthy and we have supported several special projects (see below). Our educational programs continue to flourish, the number of abstracts submitted continues to grow, and the Academy is accepting more of our suggested programming for its general audience.

Brahm Goldstein, our Program Chair, submits suggestions for both Spring and Fall Meetings. The preliminary program content for the Annual meeting in the Fall is printed in this newsletter.

In addition, abstracts are now published in Pediatrics, offering extra incentive for submitting abstracts to our Section programming. We also continue to award the best in-training, clinical and scientific abstracts.

Our Distinguished Career Award, funded this year by Nelcor, was given to John Downes,M.D. Two Critical Care Research Grants of $5,000 each were also awarded in 1996. After much discussion, the Committee decided to award one $10,000 grant in 1997. All applicants must be Section members.

The Section is currently involved in planning a CME Course with the Academy staff. Survival Strategies: Critically Ill Children in a Managed Care Environment is scheduled for April 4, 5, and 6, 1997.

A report on the Future of Pediatric Education II project was presented. This project is a three year initiative to develop a report which will include recommendations that will shape the future of pediatric education, financing and training and therefore, the future of pediatric care and delivery. Five key areas have been determined: education of the pediatrician, pediatric workforce, financing of GME, pediatric generalists of the future and pediatric subspecialists of the future. As a part of this project, there will be a survey of Sections. Curt Steinhart and David Jardine will represent the Section on critical care specific questions. The Section will attempt to survey all board certified pediatric intensivists, not only Section members.

My term as Chair of the Section officially ended with the end of the Annual Meeting. I have enjoyed my term and wish to thank all who have volunteered (or have allowed me to volunteer them) to help in these past few years!! I will continue to represent the Section as the new Medical Representative on the Council on Sections management Committee (as well as the interests of the other medical Sections). Thank you all for the opportunity to serve you and the Section!!.

Kristan Outwater

New Investigator Awards: Grant Applications

This is the second year that the Section on Critical Care will fund a New Investigator Research Award. This year's award will be for $10,000. The award is available to individuals during the pediatric critical care to pediatric critical care. The award, which is competitive, will provide support to an individual who has demonstrated aptitude for clinical or basic science research and who presents a sound plan of investigation. Dan Notterman, MD, FAAP, FCCM will be chair of the subcommittee reviewing grant applications.

The award will be judged on the following criteria: scientific merit, clarity of the presentation, likelihood of productivity by the investigator, sponsor's evidence of appropriate academic environment, and relevance to critical care.

1997 Fall Scientific Program Schedule: New Orleans, LA

In addition to our scientific abstract sessions, we will be having two 1/2 day scientific sessions next Fall. The preliminary titles and descriptions of the sessions are as follows:

1. Title: Mock Trials for the Practitioner: Keys to Criminal, Medical Malpractice, and Expert Witness Testimony

Description: This three hour session provides a live demonstration of medical-legal issues confronted by the pediatrician and pediatric subspecialist. Two expert trial attorneys, a State Supreme Court Justice, and physician-actors use real cases in a simulated courtroom to demonstrate key issues in criminal, medical malpractice, and expert witness testimony. This demonstration has received the highest critical acclaim from institutions throughout the country and provides invaluable information in an unforgettable manner.

This session will be unique in that it is a Section Program for the Practitioner meaning that the general attendees as well as members from our session will be able to attend. Fernando Stein, MD will be the lead in organizing this presentation and it promises to be extraordinary.

2. Title: Symposium on Computer Technology in Critical Care and Transport Medicine

Primary Topic Area: Advances in Computer Technology

Description: These three or four 45-60 minute didactic lectures/panel discussions will compose a 1/2 day combined session and cover exciting new advances in the application of computer technology to critical care and transport medicine. Topics include: Telemedicine, Computerized Databases, Education, and Use of the Internet/Web.

Once again, this session is not what we have done in the past. It will be a 3-way joint educational program for 1/2 day with the Sections on Computer Medicine and Transport. It also promises to be very informative and entertaining.

Editorial from the Incoming Chair - Tim Yeh, MD, FAAP

In the last five years the Critical Care Section of the American Academy of Pediatrics has seen a number of significant changes in both its activities and its impact. The Section has grown in both size and participation of its members. The educational programs of the Section have been excellent and the quality of the abstracts submitted continues to improve. This past year, for the first time, the Sections' abstracts were published in Pediatrics and this should attract even more high quality submissions. Another one of the benefits of a growing Section is that a variety of educational programs become possible. Jointly sponsored programs with other Sections that have similar interests as well as an increasing number of presentations in the Annual Meeting General Program have enhanced the visibility and influence of our Section. In addition, the dues paid by Section members have made it possible to sponsor our first Investigator's Grants as well as the Distinguished Career Awards.

Individual efforts in the Section cannot go unrecognized. Kristen Outwater has been an energetic and dedicated Chairperson and will continue have an active role as an Ex-Officio member of the Executive Committee. She was instrumental in the revision of recommendations for the use of ribavirin in bronchiolitis as well as planning the upcoming meeting on Survival Strategies: Critically Ill Children in Managed Care. Each of the Educational Program Chairs have expended considerable time and effort in putting together high quality programs for the Section. The elected members of the Executive Committee as well as the Ex-Officio members continue to provide the framework for moving the Section forward. Tim Timmons and Brahm Goldstein have been most diligent in putting together the Section Newsletter and keeping us all informed about the activities of the group. Jeff Rubenstein has chaired a group to look at RBRVS issues for pediatric critical care. Finally, the members of the Section have sustained its viability through their work on individual projects as well as their continued support and interest.

The future of the Section looks promising. We can no longer think of ourselves as an isolated group. Many members of the Section participate in other important organizations and activities including the Society of Critical Care Medicine, the Pediatric Critical Care Colloquium and the Pediatric Critical Care Study Group. It will be extremely important to consider the roles of all the other groups that have a potential impact on the care of critically ill and injured children in order to make the most efficient use of our time and effort. To this end, we plan to have the Chair of the Pediatric Section of SCCM report to the Executive on that Section's activities. Electronic communication, via the PICU Internet discussion group and the Pediatric Critical Care Web Page, make it possible to have current information without geographic or organizational boundaries and will play a key role in the future.

There are a number of issues that are currently of concern to the members of the Section, including the above mentioned managed care, as well as workforce needs, fellowship training (what kind and how many), the use of physician extenders, the changing role of intensivists and the continued development of guidelines and recommendations. Over the summer, the Executive Committee hopes to confer (by phone) and discuss some of these issues as well as the direction that the Section will take. Additional suggestions from the members of the Section would be appreciated. A number of you have already approached me to offer to participate and I encourage anyone who has any interest in helping with projects to contact me. Have a good year and see you at the Fall Meeting.
