SCCM Pediatric Section Internet Working Group (SCCMPIWG)


Meeting Minutes September 1997

Call to Order, Welcome

About 25 members of SCCMPIWG gathered for lunch on 9/18/97 during the PCCC in Hot Springs for a meeting chaired by Carl Weigle and Barry Markovitz. The group was reminded that, while it is officially the SCCM Pediatric Internet Work Group, it remains open to others interested in participating. The group also includes among its members the editorial board of the PedsCCM Website. Willing contributors are always welcome, and a signup sheet was circulated for updating the group's e-mail alias <sccmpiwg@mcw.edu>.

PICU list

The PICU list now has over 1700 members from approximately 40 countries, and the message rate is fairly stable at 10-15 per day. In May, the Medical College of Wisconsin upgraded the server on which Listproc runs. This has ultimately improved the stability of the service, but did introduce some problems:

  1. The WWW-based archives disappeared. This is planned to be re-instated this fall
  2. Q: Do threaded archives of case discussions need to be separated out on PedsCCM? Carl and Barry will look into this during the next year.
  3. The format for message digests became unuseable for many. This has since been fixed, and now is more flexible than previously was possible.

PedsCCM

  1. File Cabinet (was Hypertextbook)
    • This resource needs more original material, and needs people to send in links to other resources they find!
    • Some examples of new material there: Evidence Based Medicine resources, Jim Fackler's PowerPoint presentation on ECMO for ARDS.
  2. Rare Case registry (K Biagas, editor)
    • This section was intended to present interesting case discussions with peer review and editorial input, in a multimedia format.
    • Technical help is available for authors with multimedia input to offer.
    • PedsCCM can provide a discussant if needed, if someone has a case to present but is not ready to write the discussion.
    • Q: Turnaround time? A: >= 4-5 weeks for editorial input back to KB, then a hour to publish.
    • There is a problem with perceived lack of academic credit. Good submissions up front will enhance the prestige of the site, and encourage further submissions, etc.
      • Q: Citations by Medline, like online edition of Pediatrics? A: would take a while to get indexed, if ever.
      • Q: would this preclude print pub? A: Yes, publication is publication, as long as minimum standards are met.
      • Note: Copyright resides with author, not PedsCCM. So could publish elsewhere,but the 2nd organ might refuse it because of pre-publication. Even so, because the academic credit for case reports is low (even in print), we should encourage fellows and residents to publish these on PedsCCM.
  3. Online journal (the EB Journal Club, A. Randolph and B. Markovitz, editors)
    • The Journal of PCC-update: There has been a lack of interest on the PICU list, possibly due to a lack of need for additional primary publication venue.
    • Our vision: the EB Journal Club would evolve into a formal publication, either independent, or linked to J Peds, Peds, or other.
    • At the fellowship directors meeting (later) there was an agreement to encourage each fellowship to use EB Journal Club templates to critically review relevant literature and send in those reviews to us for publication in the EB Journal Club
    • Constancy of data?-in future may need to house the PedsCCM files in a duplicate site
    • Sponsoring organization? None have emerged clearly. PedsCCM is technically "sponsored" by the peds section of SCCM. Tacit support by AAP critical care section evidenced by publication of their newsletter (see below). SCCM maybe somehow someday? The AAP has its own e-journal. No independent foundation seems likely; there was a general lack of interest in last year's proposal
    • Arno: it comes down to money - journal prices are rising, and PedsCCM may not be able to remain free if the concept is to be sold to a publisher. Barry: charging seems very different from Internet history, although BioMedNet is charging >$200/yr fo bimonthly print plus online journal. Is this a lot more than costs? Perhaps academic centers should not pay third parties to publish our work - we should band together and perhaps a few hundred dollars per college would allow us to maintain publication of a full peer reviewed journal. Should we approach SCCM more aggressively for support?
    • Barry later spoke with K Outwater and Tim Yeh about AAP crit care section support of PedsCCM - they are interested, and may invite Barry to present the idea to the section at the AAP meeting in New Orleans in Oct/November.
  4. All-Net Pediatric Critical CareTextbook (J DiCarlo)
    • This resource is available in Russian, Spanish, and English.
    • There is a consistent format for various topics.
    • It is not yet linked to PedsCCM.
    • It is different from The File Cabinet, which is a potpourri of files and links, without consistent look and feel, while the All-Net PCC Textbook is very consistent throughout.
  5. Relationship with PCCC
    • Plan to keep publishing abstracts and ads, programs.
  6. Clinical Database(s)
    • Can we use the WWW to consolidate small series into large collection, as a health services research project? Ramesh Sachdeva interested, discussing with Barry.
    • An example would be consolidation of the large number of small reports of various PICU experiences with iNO.
