PICU Attending Progress Note Templates ReadMe
These two Microsoft Word (or Rich Text Format) files may be used or modified to serve as templates for a Pediatric Intensive Care Unit Attending Physician to create daily patient progress notes. These were created by Gene Fisher, M.D., Pediatric Critical Care Fellowship Director at the Medical College of Georgia, are are being made available "as is" with no guarantees of accuracy, comprehensiveness, or applicability. They are free to download at no cost.
- Available by direct download as: Progress_Note_v3.doc (28K) and Progress_Note_v4.doc (36K).
- Also in Rich Text Format as Progress_Note_v3.rtf (16K) and Progress_Note_v4.rtf (16K).
N.B. With Progress_Note_v3.doc, highlighted fields can have drop-down choices if locked as a form, then opened in Word; instructions to follow shortly...
